  • You need the following digital documents:

    • A recent passport-size color photograph (.JPEG format)
    • Scanned copies of both sides of your valid NIC
    • A scanned copy of the completed Life Certificate
    • A scanned copy of your dual citizenship certificate (if applicable).
  • No, all documents must be uploaded digitally during the online application process.

  • A Life Certificate is a document that verifies your personal information and current status. Print, complete, and have it certified by authorized personnel before scanning and uploading.

  • Payments can be made via the online portal using a local or international credit/debit card.

    • In Sri Lanka: A Director of the Hospital, Head of the Institution, or your local Grama Niladhari.
    • Abroad: A Director of the Hospital, Head of the Institution, or a Sri Lankan Embassy/Consulate official.
    • Personal Information (photo, SLMC registration number, NIC, citizenship status, full name, address, and signature).
    • Certification details from a JP, Commissioner of Oaths, or Attorney at Law.
    • Certification of being alive, completed by authorized personnel.
  • You must upload a scanned copy of your dual citizenship certificate and specify the other country of your citizenship.

  • Yes, retain the original Life Certificate until 31.12.2029 in case it is requested by SLMC.

  • Yes, you will receive confirmation via the online portal, email or SMS after successfully completing the process.

  • Ensure your NIC details on the Life Certificate match the uploaded scanned copy. Contact SLMC if discrepancies persist.

  • Yes, but certifications may need to be done by authorized personnel in your country of residence, such as a hospital director or consulate official.
